Bolt fell out!!

The bolt that holds the pedal mount/brace to the transmission on my cargo CCKW fell out! I lost virtually all of the GO in the tranny. So far, it appears that I caught it in time.
The bolt had been in there for ~14 years; since I replaced the engine in the early 2000's.
I guess the lession is to either replace or check the lock washers very closely if you disassemble something!
